Our Team Approach
We strive to achieve excellence in service delivery through adherence to the code of ethics and best practices within Community Health Centre evidence-based practice.
Our team is client-centered, trauma-informed, strength-based, and utilizes a harm reduction approach. We adhere to Anti-Racism, Anti-Oppressive frameworks, and a Collaborative framework to address healthcare access issues and remove barriers, positively impacting health outcomes.

Health and Wellness
Our teams, including community health workers, early child development workers, health and wellness counselors, and social workers, provide comprehensive services to address the social determinants of health. The Health and Wellness Team offers health promotion, chronic disease prevention, and specialized services like sickle cell disease programming, an ID clinic, and housing support. Early Child Development Workers focus on child development and parenting support, while our Foodshare services tackle food insecurity. The Social Work Team is dedicated to mental well-being, offering health promotion programs, drop-in counseling sessions, and case management to help clients navigate the healthcare and social service systems. We are committed to improving health outcomes for the diverse communities we serve.

Settlement
Our team, comprised of access facilitators, health settlement counselors, youth wellness workers, and newcomer orientation counselors, is dedicated to helping newcomers integrate into our community. Our Health and Newcomer Settlement Counselors provide immigration services and create individualized settlement plans, assisting with housing, overcoming language barriers, and addressing food insecurity. Youth Wellness Counselors work to build a healthy future for our community’s youth, while Access Facilitators connect people with resources and foster partnerships. We collaborate closely with other programs and build strong partnerships within Hamilton to support our community.

Black Health Team
Our team, comprised of social workers, health and wellness counselors, and case management workers, is dedicated to serving the Black community through the Black Health Program at Hamilton Urban Core Community Health Centre (HUCCHC). Recognizing the disparities faced by Black people in accessing health services, HUCCHC collaborates with community health organizations to address social determinants of health and combat Anti-Black Racism in healthcare. The program’s mission is to improve health and wellness by providing culturally safe, accessible, and effective community-based services through an Africentric lens. Our vision is for Black individuals to have equitable access to healthcare and support, eliminating disparities in health, education, and child welfare sectors to achieve better health outcomes for all.
