Position Description:
The Black Health Community Health Promoter plays a vital role in advancing Black Health outcomes by improving the accessibility, cultural relevance, and responsiveness of health services for Black communities, particularly those disproportionately impacted by systemic health inequities. This position focuses on identifying health disparities, recommending and supporting targeted interventions, and embedding anti-racist, anti-oppressive, and culturally responsive practices into all aspects of health promotion and service delivery.
The Black Health Community Health Promoter is responsible for delivering education and prevention strategies, implementing Black Health social prescribing initiatives, providing service navigation, leading community outreach, and supporting program development, implementation, needs assessments, and program evaluation activities.
Through education, prevention, community engagement, and collaborative program development, the Black Health Community Health Promoter works closely with interdisciplinary teams and community partners to address and reduce inequities across the social determinants of health, ultimately supporting improved health outcomes and strengthening wellness within Black communities.
Accountability:
- The Black Health Community Health Promoter reports to the Manger, Community Health Promotion, with overall accountability to the Executive Director.
Key Responsibilities:
- Provide community-based education and support by designing and implementing health programs, advocating for practice changes that promote equity, and collaborating with diverse partners to achieve meaningful, sustainable improvements in health outcomes for Black communities.
- Proactively identifies and addresses individual and systemic barriers to care, championing equitable, inclusive, and culturally safe access to health services for historically underserved and marginalized communities.
- Advocate for enhanced access to health services and equity in healthcare for Black communities by addressing barriers such as racism, discrimination, and limited resources.
- Works in partnership with community-based organizations and internal stakeholders to assess the health needs, strengths, and priorities of clients and communities, with particular attention to priority and emerging populations.
- Develop and deliver culturally responsive education, prevention, health promotion, and wellness programs to support the mental health, chronic disease management, and overall well-being of Black individuals, families, children, and youth.
- Facilitate service navigation and social prescribing, connecting Black families to culturally safe health services, chronic disease management programs, Black therapists and counsellors, community supports, and wellness resources.
- Implement chronic disease management education and self-management supports for Black individuals living with or at risk of conditions such as diabetes, hypertension, asthma, and heart disease.
- Conduct targeted health promotion activities aimed at reducing health disparities and strengthening protective factors for Black children, youth, and families.
- Conduct outreach activities in schools, education support centers, community hubs, recreation centers, faith-based organizations, and housing communities to enhance visibility and participation in health services.
- Facilitate connections to culturally affirming healthcare and mental health services through referrals, system navigation, advocacy, and social prescribing.
- Collaborate with interdisciplinary teams, clinical staff, and community organizations to deliver holistic, wraparound health supports that integrate mental, physical, and social health care for Black populations.
- Conduct community needs assessments, focus groups, surveys, and stakeholder consultations to identify emerging health needs, gaps in services, and barriers to access.
- Support program development and contribute to grant applications, funding proposals, and sustainability strategies for culturally responsive Black health initiatives.
- Design, implement, and evaluate educational programs, workshops, and wellness events focused on health literacy, chronic disease prevention, health promotion, mental health, and stigma reduction.
- Collect and analyze program data to monitor service impact, report outcomes to funders and internal leadership, and support continuous quality improvement.
- Embed anti-Black racism, anti-oppression, health equity, and trauma-informed principles into all aspects of program delivery, outreach, and client engagement.
- Represent the organization in community coalitions, working groups, and partnerships that advance Black health, mental health equity, chronic disease prevention, and social determinants of health.
- Foster trust and cultivate strong relationships with community members by actively listening to their concerns, understanding their needs, and ensuring that health programs are both culturally relevant and responsive to their unique circumstances.
- Work within an anti-racism, anti-oppression, culturally sensitive, language appropriate framework.
- Work as part of the interprofessional team at the Centre and adhere to the Centre’s core competencies and the CHC Model of Care.
- Participate in Centre-wide activities such as the annual general meeting, client-centered activities, annual events and all other special events and activities related to Hamilton Urban Core.
- Adhere to organizational policies, professional standards, and ethical guidelines in all aspects of client care and documentation
Other:
- Provide mentorship and supervision to students, interns, and new staff.
- Engage in ongoing professional development, including training, literature review, and peer collaboration.
- Support Hamilton Urban Core CHC-wide initiatives, including annual events, advocacy efforts, and client-centered programming.

Qualifications:
- Master’s degree in Health Sciences, preferably Public Health, Health Promotion, or Social Work. o Experience in Black Health community engagement and outreach.
- Experience in health education and promotion utilizing culturally appropriate materials and engaging delivery methods.
- Deep connection to, and knowledge of, Black communities, including cultural traditions, lived experiences, and systemic barriers to healthcare access.
- Experience supporting individuals in accessing and navigating healthcare services, including referrals to appropriate community and clinical resources.
- Familiarity with equitable, culturally responsive approaches when working with diverse populations, particularly Black communities.
- Experience in collecting and analyzing data to understand health trends and inform targeted interventions.
- Demonstrated cultural sensitivity, with an understanding and respect for Black cultures, values, and beliefs.
- Strong communication skills, with the ability to engage effectively with community members, healthcare providers, and stakeholders using clear, respectful, and culturally appropriate language.
- Understanding of the social, economic, and political determinants of health and their impact on the well-being of Black populations.
- Proven ability to advocate for community needs and take on leadership roles that promote health equity, empowerment, and positive change.
- Effective problem-solving and critical thinking skills, with the ability to identify problems, analyze data, and develop solutions that address community health needs.
- Ability to manage multiple tasks and prioritize responsibilities effectively in a fast paced environment.
- Demonstrated commitment to health equity, social justice, and culturally responsive service delivery.
- Commitment to anti-Black racism, anti-racism, anti-oppression, antidiscrimination, equity, and culturally grounded models of care.
