Hamilton Urban Core Community Health Centre (HUCCHC) has been addressing health inequities such as poverty, racism, discrimination, unemployment & homelessness, faced by community members,
neighbours, friends and families in Hamilton’s city core for nearly 30 years. The Centre addresses the complex needs of vulnerable, racialized, and marginalized communities by providing services such as primary healthcare, community health promotion & harm reduction services.
The Black Health Cultural Community Ambassador functions as a member of the Black Health team under the Community Health Promotion department in the provision of multidisciplinary health care services to residents of the community, with a deep understanding and appreciation of diverse cultures in the Black community. The Black Health Cultural Ambassador will be responsible for promoting cultural awareness and sensitivity within our organization and the community. Working closely with different teams to develop and implement programs and initiatives that celebrate the diverse Black culture and fosters an inclusive environment.
Working with the Centre’s Black clients, the Black Health Cultural Community Ambassador will provide health care services and participate in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community.
Key Responsibilities:
- Use an anti-oppressive and Africentric practice approach
- Link Black clients and family members to mental health and counselling supports
- Support mental health needs of Black children living with sickle cell disease
- Develop working relationships and rapport with community service providers in providing culturally adaptive language & services which consider the experiences of the African, Caribbean, and Black (ACB) community around racial trauma, racial gaslighting
- Develop and maintain a flexible work schedule that accommodates the needs of the community as approved by your manager. Ability to work flexible hours including evening and weekends
- Work closely with all teams at the Centre; Black Health, Consumption and Treatment Services, Community Health Programs, Primary Health Care and Prescribed Alternatives and Outreach Team to promote cultural awareness and sensitivity within the organization
- Highly developed participatory group facilitation skills
- Build community connections and partnerships with community agencies
- Bridge hospital and community services to deliver holistic care, addressing key social determinants of health for Black clients
- Ensure clients receive ongoing support across the continuum of care, including healthcare, social services, and community supports
- Ensure all interventions and programs are grounded in trauma-informed care, anti-racism, and cultural safety
- Provide culturally competent support to Black individuals and families, ensuring they feel respected and understood within the healthcare system
- Foster strong relationships with Black community organizations and stakeholders to enhance service delivery
- Actively engage Black families in health and wellness programs, promoting mental health awareness and access to culturally appropriate resources
Qualifications:
- Bachelor’s degree in cultural studies, community development, psychology, social work, Black studies, social sciences, or a related field
- Certification or training in trauma-informed care, anti-racism, and cultural safety is highly desirable
- Minimum of 3 years of experience in a similar role
- Experience in collaborating with community organizations, healthcare providers, and multidisciplinary teams
- Familiarity with the social determinants of health, particularly in relation to the Black community
- Demonstrated commitment to and lived experience of anti-Black racism and Africentric values and principles
- Excellent interpersonal and communication skills, with the ability to build strong relationships with diverse communities and stakeholders
- Strong advocacy skills, with the ability to navigate and bridge between hospital and community- based healthcare systems
- A deep fundamental and comprehensive knowledge of the unique challenges faced by African Canadian and Caribbean individuals across the social determinants of health
- A commitment to interdisciplinary team approach to coordination of health and social services
- Excellent communication and interpersonal skills
- Ability to work collaboratively with a wide range of stakeholders
- Experience working with volunteers
- Proactive and creative approach to problem-solving.
- Strong organizational and project management skills.
- Experience in organizing and coordinating events.
- Ability to develop and deliver training programs.
- Knowledge of current trends and best practices in cultural diversity.
- Experience in building and maintaining relationships with community groups.
- Ability to advocate for the needs and interests of cultural groups.
- Strong presentation and public speaking skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office and other relevant software.
- Ability to handle sensitive and confidential information.
- Strong analytical and problem-solving skills.
- Commitment to promoting cultural understanding and appreciation.
- Develop and maintain an onsite flexible work schedule that accommodates the needs of the community. Ability to work flexible hours including evening and weekends
- Fluency in French, is a significant asset
