Foot Care Registered Practical Nurse (0.6 FTE/ 3 days a week)
Salary Range ($47,000.00 – $53,140.56 prorated to FTE)
Hamilton Urban Core Community Health Centre (HUCCHC) has been addressing health inequities such as poverty, racism, discrimination, unemployment & homelessness, faced by community members, neighbours, friends and families in Hamilton’s city core for nearly 30 years. The Centre addresses the complex needs of vulnerable, racialized, and marginalized communities by providing services such as primary healthcare, community health promotion & harm reduction services.
The Foot Care Registered Practical Nurse (RPN) is an essential member of the client-centered, interprofessional, multidisciplinary team of health providers at Hamilton Urban Core. The Foot Care RPN will work with other members of the Centre’s teams to effectively and efficiently serve clients and communities using responsive and innovative approaches to services and service delivery that is framed by health equity and the social determinants of health.
Key Responsibilities:
1. Work with the primary health care team to develop, implement and evaluate foot care health promotion activities in the community by identifying health needs and increasing awareness of foot care services.
2. Perform clinical duties by assessing for routine foot health and illness care, treating and preventing disease, dysfunction or disorders of the foot by therapeutic, surgical, palliative or orthotic means within the scope of the license.
3. Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate clinical records, participating in chart reviews and discussions of problem cases, answering client enquiries and directing clients to or providing appropriate treatment.
4. Maintain accurate, current client records, data and information and electronically encounter client services in accordance with the Centre’s policies, best practices and standards of good practice. Complete and submit reports as required.
5. Assist in the planning and programming of the Centre’s health services by participating on committees and by sharing information obtained through client encounters about perceived health and problems in the community.
6. Identify areas where the development of protocols and procedures is needed to improve client services or to promote more effective staff functioning.
7. Prepare orthotics as appropriate for clients
8. Order supplies as needed . Maintain and develop professional competence.
9. Where possible, assist in the education of staff and supervise procedures performed students of foot care and orthotic treatments.
10. Actively participate in the Centre’s quality improvement and quality assurance plans. Participate in meetings of the Centre as appropriate such as Team, Case Conference and Staff meetings.
11. Engage and provide supervision to graduates, students and/or learners as appropriate and in accordance with the Centre’s standards and commitment to learning.
12. Work within an anti-racism, anti-oppression, culturally sensitive, language appropriate framework.
Qualifications:
1. Registered Practical nursing diploma
2. Current registration with and good standing with the College of Nursing of Ontario without limitations
3. Three to five years foot care experience in a community health or hospital setting
4. Knowledge in providing foot care
5. Strong clinical and assessment skills
6. Flexible to meet changing team/service needs
7. Proficiency in the use of computers and various software applications
8. Experience with and sensitivity to issues impacting clients and communities accessing the Centre in particular marginalized populations and individuals with complex health issues
9. Proven ability to effectively interact within a multi-disciplinary team
10. Good problem-solving skills
11. Excellent verbal and written communication skills essential for medical reports and educational and promotional activities.
12. Knowledge of the community health centre sector is an asset.
