Hiring Range $68,000- $76,000
Hamilton Urban Core Community Health Centre (HUCCHC) has been addressing health inequities such as poverty, racism, discrimination, unemployment & homelessness, faced by
community members, neighbours, friends and families in Hamilton’s city core for nearly 30 years. The Centre addresses the complex needs of vulnerable, racialized, and marginalized communities by providing services such as primary healthcare, community health promotion & harm reduction services.
Reporting to the Senior Manager, Integrated Care and Client Experience, the Manager works within the context of the Centre’s mission and strategic priorities to manage and provide leadership in the planning, delivery and evaluation of integrated and responsive health services. The Manager is responsible for the overall management and direct supervision of the Team. Working as a key member of the Management Team the Manager collaborates internally and externally to ensure a highly functioning, inter-professional team and ongoing quality services.
The Manager may also be a Registered Nurse who may also serve as the Responsible Person in Charge (RPIC).
Key Responsibilities:
- Manage the assigned human resources, foster inter-professional team approaches and optimize the skills and knowledge of providers to ensure quality service
- Ensure continuous quality improvements and adherence to best practices or leading practices related to primary health care services and health promoting programs particularly in relationship to the Centre’s priority populations
- Participate in operational planning process and management of the Centre’s resources particularly related to consumption and treatment services and any assigned portfolios.
- Work in tandem internally across teams and externally across agencies and service providers to develop or enhance service collaborations and partnerships to connect people with primary health care services, community health services and prescribed alternatives services to expand the range of service for clients
- Provide leadership and support for direct reports to ensure the effective use of EMR and other health technologies
- Implement community engagement measures
- Provide leadership and support to direct reports and other members of the staff team, ensuring alignment with organizational goals and objectives, and providing guidance, support, and performance evaluation
- Coach and mentor direct reports
- Assign and adjust work assignments and staffing schedules, ensuring service levels and staffing requirements are met
- Develop, adapt, implement, and document mechanisms to ensure effective service delivery and professional practice including policies and procedures for day-to-day service and program functions
- Oversee scheduling processes, ensuring the effective use of staff and facility resources
- Monitor workload and performance indicators, particularly those identified in accountability agreements with funders
- Support, coach, and supervise assigned staff, including the development and monitoring of goals and individual work plan
- Complete regular performance appraisals for all direct reports
- Support in developing and monitoring operating budgets and ensuring expenditures remain within budget
- Review and approve expenditures within assigned budgets
- Participate in policy development and review activities
- Participate in Centre-wide risk management initiatives
- Participate in accreditation processes and provide leadership in areas related to the assigned program areas
- Collaborate with the leadership to ensure achievement of the Centre’s performance and quality improvement targets and operational priorities. Actively support the Centre’s core competencies and the culture of equity consistent with the Centre’s mission and vision
- Work during both core and extended hours (evenings/weekends as required) to accommodate the needs of the Centre’s clients
Qualifications:
- A Certificate of competence from the Ontario College of Nurses is a strong asset
- Post- secondary degree in nursing, public health, social work, counselling psychology, addictions and mental health, health sciences, etc. (Master’s Degree is a strong asset)
- Experience in harm reduction service delivery is a strong asset
- Thorough knowledge of and proficiency in program planning, implementation, and evaluation
- Three to five years’ management experience in a community-based, human or health services organization
- Excellent knowledge of harm reduction, addiction, mental health and homelessness
- Demonstrated commitment to and knowledge of community-based health care
- Excellent knowledge of community health practices and related policies and procedures
- Experience and understanding of the health needs of individuals/families in the inner city
- Good communication and interpersonal skills, with a demonstrated ability to supervise and direct staff to ensure an effective and efficient working environment
- Previous experience and demonstrated ability in budget preparation and proposal writing
- Flexible with experience leading a multi-disciplinary team and facilitating effective team planning
- Excellent oral and written communication skills.
- Demonstrated analytical and organizational skills.
- Commitment to the anti-racism and anti-oppression principles at Hamilton Urban Core
- Ability to collect and analyze data, including familiarity with computer software programs
- Ability to be flexible and adaptable to the daily changing needs within the community and to handle challenges with compassion and resolve.
