What is the Hamilton HART Hub?

The Hamilton HART Hub is a low-barrier, trauma-informed, and culturally responsive health and recovery hub operated by Hamilton Urban Core Community Health Centre in partnership with local service delivery organizations.

It brings together substance use health, mental health care, primary care, case management, housing supports, systems navigation, health promotion, and wraparound social supports for people with complex needs.

The Hub at 430 Cannon Street East operates as the walk-in access point: no appointment, no ID, and no referral required. The Residential Treatment and Recovery Program at 276 Aberdeen Avenue is part of the broader HART Hub, but there is no direct walk-in access at Aberdeen.

Residential Treatment Program

The Residential Treatment and Recovery Program is part of the broader Hamilton HART Hub. It is for adults who are ready for structured, intensive residential treatment in an abstinence-based environment.

  • There is no walk-in access at the Aberdeen residential site

  • Must be 18 years or older and a resident of Hamilton, Ontario
  • Voluntary, abstinence-based program lasting 8 to 12 weeks

Who is eligible?
Adults 18 years and older who are residents of Hamilton, Ontario, facing substance use and mental health challenges, and who are ready for and committed to a voluntary, abstinence-based recovery program.
How long is the program?
8 to 12 weeks, depending on individual progress and needs.
Is the program voluntary?
Yes. Participation is entirely voluntary. Residents may choose to leave at any time, and safe discharge support is available.
What are the facility protocols?
24-hour on-site staffing and security. Controlled building access. Residents use the interior courtyard for recreational time. Off-site appointments are accompanied by staff. The building is drug-free, alcohol-free, and smoke-free. A designated outdoor smoking area is in the interior courtyard.
What is the Neighbourhood Liaison Table?
A standing forum for dialogue between Hamilton Urban Core and local residents. It meets regularly to provide updates on the Residential Treatment and Recovery Program, address community questions, and support respectful, ongoing communication between the program and the neighbourhood.
Who governs and oversees the program?
The Hamilton HART Hub is operated by Hamilton Urban Core Community Health Centre, a non-profit governed by a Board of Directors. It is publicly funded by the Ontario Ministry of Health and subject to regular government oversight, Board accountability, and independent financial auditing.
