All About Hamilton Urban Core Community Health Centre

Hamilton Urban Core Community Health Centre (HUCCHC) is a community-based, non-profit organization that has been serving downtown Hamilton for nearly 30 years. The Centre provides health and social services to individuals and families who often face barriers to care.

With a focus on dignity, equity, and community, HUCCHC creates a safe space for those navigating poverty, trauma, homelessness, addiction, and other challenges.

What We Offer

Hamilton Urban Core provides a wide range of programs and services that support the health and well-being of the whole person. These include:

• Primary care services delivered by doctors, nurses, and nurse practitioners.

  

• Mental health and addiction support, including counseling and harm reduction.

• Dental and foot care

• Newcomer and refugee support

• Seniors’ wellness programs

• Youth programming and community kitchen activities

All services are offered in a welcoming environment that respects each person’s background, identity, and lived experience.

How We Work

HUCCHC uses a team-based, client-centered approach to care. That means looking beyond symptoms and treating the full person; which includes the mind, body, and social situation. The team includes healthcare professionals, social workers, peer workers, and community advocates who work together to support clients with empathy and consistency.

The Centre follows a rights-based model of care that recognizes the role of social factors like housing, income, and education in shaping health. Every program is designed to be inclusive, trauma-informed, and responsive to community needs.

A New Purpose-Built Space

To better serve the community, Hamilton Urban Core is moving to a new facility at 430 Cannon Street East. This purpose-built space will allow for expanded services, increased accessibility, and a more comfortable and private experience for clients. The new location includes clinical rooms, group spaces, a teaching kitchen, and areas designed for community gathering.

This move marks a new chapter for HUCCHC, with the goal of serving more people and deepening the impact of its work across Hamilton’s urban core.
