Hamilton Urban Core offers a variety of programs to promote health & wellness, prevent and manage chronic disease.

Life Lessons Parenting Program

The Life Lessons Parenting Program is facilitated by an Early Childhood Development Specialist who provides:

  • Parenting skills
  • Newcomer parenting
  • Resources
  • Peer support
  • Networking
  • Socializing

Inner City Women's Wellness

Community issues have a direct impact on women’s health for marginalized, racialized, immigrant, refugee, and poor women living in the inner city. 

In a  weekly group setting, women gain access to resources and information related to women’s health issues.

Anger Management for Women

In this 8-week series, a Social Worker facilitates this program that teaches women skills to regulate emotions and how to socialize.

The group also covers topics related to general health and wellbeing and provides resources for future referral.

Anger Management for Men

In this 10-week session, men learn skills to manage their emotions.

Men are encouraged to socialize and participate in discussion topics related to men’s health and well-being,

Additional resources are provided by the Community Health Worker who facilitates the session. 

Life After Fifty-five (L.A.F.F.ers)

This weekly group gathers individuals over the age of fifty-five to socialize and reduce isolation.

The Conversation Circle supports newcomers to improve English language skills while everyone benefits from greater cultural awareness.

Art from the Heart

This weekly support group, facilitated by a Social Worker, uses art as a means to support good physical and mental health by promoting overall social engagement.

Blood Pressure Clinic

A safe environment and forum for discussion where participants meet to enjoy a nutritiouos meal.

Participants can access blood pressure monitoring, education and resources.

Facilitated by a Registered Nurse. Referrals to other services provided as required. 

EARLY STEPS

The Early Steps program is facilitated by an Early Child Development Specialist 

The program offers family, group or individual support regarding early stages of development for children aged 2 to 6 years.

Features of the program include:

  • Child development, including milestones
  • Child-friendly activities
  • Reading program
  • Peer Support
  • Resource Sharing

Foot Clinic

Registered Nurses and Chiropodists collaborate to provide foot care to maintain foot health.

All services are available to clients free-of-charge, and include:

  • Removal or corns and callouses 
  • Treatment of toenail problems
  • Routine nail care for high risk clients
  • Wound and ulcer care
  • Custom-made foot orthotics & show modifications
  • In-office surgical procedures

 

Girl Talk

Girl Talk is a teen-based program that meets weekly. 

Focused on self-awareness, teens learn skills to manage stress and peer pressure.

The Child Development Speicalist facilitating the program also covers health issues common among teenage girls

Happy Hearts

Healthy lifestyle choices are essential to maintain a healthy, happy heart. 

This weekly program, offered as part of a group setting, promotes awareness to make healthy lifestyle choices, focused on heart health education.

This program offers guest speakers who address food access and physical exercise.

Men's Circle

Weekly meetings of the Men’s Group centre around socialization and discussion topics related to men’s health and well-being.

The sessions, facilitated by a Community Health Worker provides participants with resources to reinforce their learning. 

Oral Health

The Community Oral Health Program provides clients with care and services to manage dental hygiene and manage overall oral health.

A Self-Initiated Hygienist provides the following services:

  • Cleaning & polishing
  • Pits & fissures sealant
  • Topical fluoride application
  • Desensitizing agent treatment
  • Nutritional counselling
  • Check-ups – advice, referrals, follow-up appointments
  • Education programs in schools, shelters & workplaces

Seniors

Basic education and information on the prevention and management of chronic disease. 

Weekly meetings over 8 to 10 weeks, including workshops to learn lifestyle modifications, including physical activity, weight reduction, blood pressure and blood sugar control, healthy eating and stress management. 

Serenity In Motion

This women’s group is hosted by a Health & Wellness Counsellor and meets weekly to discuss topics related to women’s health and well-being. 

Women also have an opportunity to share resources and socialize.

Streetwise

This drop-in group program meets on the first Tuesday of each month.

Participants receive education and resources to support safe injection practices and healthy sex practices.

Through a series of interactive workshops, the program is greared toward greater awareness to make healthier choices.

Peer Volunteers, Guest Speakers and Community Health Workers collaborate on this initiative.
