Integrated Primary Healthcare

Primary healthcare: a right, not a privilege. Accessible, comprehensive care for all.

Hamilton Urban Core offers a wide variety of Primary health Care services including specialty services such as: Chiropody; Dental; Palliative Care, Chiropractic clinic and Diabetes Services.

The Primary healthcare team is a part of the Greater Hamilton Health Network (GHHN) OHT Primary Care Network and works with varied health system partners to provide needed care for patients; working closely in collaboration with other predominant community services and health networks to provide patients with most appropriate team members to care for their specific health concerns.

Community Health Promotion Programs

At Hamilton Urban Core Community Centre, we believe that good health is essential for a happy and fulfilling life.

We offer a range of community health programs designed to promote physical, mental, and emotional well-being for individuals and families in our community.

Substance Use Health

We take a compassionate, holistic, and client-centered approach to substance use health; meeting people where they are at, without judgment. Our services support individuals at every stage of their journey, whether they’re actively using substances, looking to reduce use, or working toward recovery.

We provide access to harm reduction education, counseling and supplies, overdose prevention, connections to healthcare, mental health, and addiction treatment, as well as comprehensive wraparound care and social support. Everyone deserves care in a safe, welcoming, and dignified environment.
