The Nurse Practitioner - Substance Use Health is a key member of the Mental Health and Addictions Treatment (MHAT) Hub, providing client-centered, trauma-informed, and culturally safe primary health care to individuals experiencing complex challenges related to substance use, homelessness, and mental health.
The Primary Health Care Manager leads and manages all aspects of programs and services in the planning, implementation and development, monitoring and evaluation for primary health care including chronic disease prevention and management, health maintenance, health promotion, health education, mental health and other health areas. The Primary Health Care Manager works within the context of the Centre’s mission and strategic priorities to manage and provide leadership in planning, delivery and evaluation of integrated and responsive health programs and services.
The Black Health – Mental Health and Wellness Counsellor, provides counselling/therapy and other therapeutic functions and participates in the development, implementation, monitoring and evaluation of programs and services for the Black community. As a member of the multidisciplinary, inter-professional team, the Black Mental Health and Wellness provides case management, individual and group-work services to Black clients.
The Black Health Social Worker plays a critical role in advancing health equity by delivering culturally responsive, trauma-informed mental health and addictions supports to Black individuals, children, youth, and families. The Social Worker is responsible for leading direct clinical service delivery, early intervention, and crisis support initiatives. This role addresses systemic barriers and inequities that Black communities face in accessing mental health services and promotes healing, resilience, and emotional well-being across the lifespan.
