Position Description:
The Substance Use Health Registered Nurse (RN) provides provision of harm reduction services to clients that access Substance Use Health, with a strong emphasis on disease prevention and health promotion. Working primarily with the Centre’s priority populations, the RN will provide primary health care services and participate in the development, implementation, monitoring and evaluation of services for individuals who use drugs.
Accountability:
The RN for Substance Use Health reports to the Program Manager for Substance Use Heath with overall accountability to the Executive Director.
Key Responsibilities:
Perform assessments, diagnosis, triage, health and harm reduction education/counseling, history taking and other clinical activities according to the PAOP clinical protocols and College of Nurses of Ontario (CNO) standards of nursing practice
Perform assessments on skin and soft tissue infections and either refer to NP for wound care or perform minor wound care as needed
Maintain accurate, current client records, chart reviews, data and information and electronically encounter client services in accordance with the Centre’s policies, best practices, and standards of good practice
Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services
Develop substance use related knowledge products to share with clients and stakeholders
Pre-drawing injectable naloxone for potential use during the shift
Running through oxygen procedure at beginning of the shift
Providing harm reduction supplies as needed
Providing support for overdose as appropriate (stimulation, PPV via BVM, naloxone, CPR if required)
Appropriately storing any substances left behind on site at end of shift and informing the Responsible Person in Charge
Ensuring the oxygen tank is off and properly secured at end of shift
Ensuring that the tracking sheet is appropriately filled throughout shift
Providing medical advice is not an expectation of the RN. However, the RN may choose to provide medical advice that is within the RN’s comfort level and their identified scope of practice
Contribute chart documentation, participate in chart reviews and case conferences and make referrals
Engage and provide supervision to graduates, students and/or learners as appropriate and in accordance with the Centre’s standards and commitment to learning
Actively participate in the Centre’s quality improvement and quality assurance plans. Participate in meetings of the Centre as appropriate such as Team, Case Conference and Staff meetings
Maintain and develop professional competence through appropriate continuing education (i.e. peer interaction, literature review, conferences, courses or educational leave)
Work within an anti-racism, anti-oppression, culturally sensitive, language appropriate framework
Work as part of the inter-professional team at the Centre and adhere to the Centre’s core competencies and the Community Health Centre Model of Health and Wellbeing.
Participate in Centre-wide activities such as the annual general meeting, client-centered activities, annual events and all other special events and activities related to Hamilton Urban Core
Qualifications:
Bachelor of Science in Nursing degree
Current registration with the College of Nurses of Ontario
Experience in a community setting or combination of community, hospital, or public health setting
Knowledge of harm reduction principles, injection drug use and associated health issues
Experience in primary care or street/outreach nursing an asset
Experience with and sensitivity to issues impacting on populations in the downtown core which may include immigrants and refugees, isolated seniors, people living in poverty, people living with mental illness, people who use drugs and/or people who are homeless or at risk of being homeless
Demonstrated experience working with people who use drugs
Understanding and ability to apply the principles of health promotion, disease / injury prevention, and determinants of health
Demonstrated ability to de-escalate conflict as an asset
Confidence in asserting and maintaining strong boundaries
Excellent clinical and problem-solving skills
Contribute to knowledge exchange products
Proficiency in the use of electronic medical records
Experience in program development, implementation monitoring and evaluation an asset
Strong oral and written communication skills
Respects and values the diversity of communities and individuals
Ability to work effectively as a member of an interdisciplinary / multidisciplinary / inter-professional team
