Our team is client-centered, trauma-informed, strength-based, and utilizes a harm reduction approach. We adhere to Anti-Racism, Anti-Oppressive frameworks, and a Collaborative framework to address healthcare access issues and remove barriers, positively impacting health outcomes. We offer a range of exceptional programs designed to help our clients, they are as below.
Our teams, including community health workers, early child development workers, health and wellness counselors, and social workers, provide comprehensive services to address the social determinants of health. The Health and Wellness Team offers health promotion, chronic disease prevention, and specialized services like sickle cell disease programming, an ID clinic, and housing support.
Early Child Development Workers focus on child development and parenting support, while our Foodshare services tackle food insecurity. The Social Work Team is dedicated to mental well-being, offering health promotion programs, drop-in counseling sessions, and case management to help clients navigate the healthcare and social service systems. We are committed to improving health outcomes for the diverse communities we serve.
Our settlement team supports newcomers in building healthy, stable lives in Hamilton. We include access facilitators, health settlement counselors, youth wellness workers, and orientation staff.
We help with immigration, housing, language barriers, and food security, offering personalized settlement plans.

Our Black Health Programme offers culturally safe, community-based care for Black individuals and families in Hamilton. The programme includes mental health services, wellness support, and system navigation, all grounded in an Africentric lens.
As part of this work, the AMANI Programme provides youth aged 12–29 with affirming mental health and substance use support through counselling, therapy, and family-focused care.
