Project Overview: HART Hub Residential Treatment and Recovery Services

About the Program

The Homeless and Addiction Recovery Treatment (HART) Hub is a provincial initiative a low barrier, trauma-informed, culturally responsive healthcare model that integrates a suite of services for clients with complex service needs such as: primary care, mental health, substance use treatment, housing with supports, and systems navigation.

The Hamilton HART Hub is led by Hamilton Urban Core Community Health Centre with the main HART Hub site located at 430 Cannon Street East. Services are colocated with partner organizations, and the Residential Treatment and Recovery Services program will operate out of 276 Aberdeen Avenue.

The Residential Treatment and Recovery Services is designed to provide integrated recovery and treatment services for individuals experiencing substance use and mental health challenges in a compassionate, safe, and supportive environment, helping individuals stabilize, build life skills, and transition successfully back into the community.

Our goal is to strengthen access to integrated care, promote recovery and wellness, and contribute to a healthier, more connected Hamilton community.

Purpose of the Project

To support individuals facing substance use challenges by offering short-term, bed-based intensive addiction treatment and supportive recovery services in a safe and structured residential setting.

The residential treatment and recovery services aims to:

  • Provide a continuum of recovery and treatment services tailored to individual needs.

  • Offer timely, trauma-informed, and evidence-based care within a welcoming and stigma-free environment.

  • Reduce barriers to treatment by creating a single point of access for supports, referrals, and resources.

  • Create a structured environment that supports the development of life skills for community reintegration.

  • Support community well-being by connecting clients with healthcare, housing, and social supports.

  • Collaborate with community partners to ensure that services are coordinated, sustainable, and culturally responsive.

Services Offered

The program will deliver a range of clinical and wraparound support services, including:

  • Abstinence-based substance use treatment and recovery services

  • Individual and group counselling and programming

  • Medical and nursing supports

  • System navigation services

  • Cultural, spiritual, and wellness supports

  • Life skills and personal capacity development

Community Engagement

Community engagement is central to the residential treatment and recovery program’s design and implementation.

  • Input from residents, partners, and local organizations is being sought through meetings, open houses, and surveys.

  • Feedback will directly inform how services are offered, how spaces are designed, and how partnerships are strengthened.

  • Ongoing opportunities will be available for residents to stay informed and involved as the project progresses.

Hamilton Urban Core Community Health Centre is committed to:

  • Sharing regular updates on our website and through email communications to our contact lists regarding program progress, milestones, and launch information.

  • Working collaboratively with stakeholders, including municipal partners and community organizations, to ensure the safety and security of residents and neighbours.

  • Establishing a Neighbourhood Liaison Table to support ongoing communication, transparency, and collaboration between the program and local residents.

  • Facilitating opportunities for community connection, such as participating in neighbourhood groups, community clean-ups, and local events (e.g., BBQs), to foster mutual understanding and inclusion.
