Position Summary
The Mobile Unit Worker plays a key role in increasing access to culturally responsive, community-based health and social services for underserved, marginalized, and equity-deserving populations. Working primarily in outreach and mobile settings, this role supports individuals and families using a holistic, trauma-informed, client-centered, anti-oppressive, and harm reduction–oriented approach.
The Mobile Unit Worker engages people where they are to reduce barriers to care and facilitate connections to primary care, health promotion, mental health and addictions supports, harm reduction services, and social supports offered through Hamilton Urban Core and partner organizations.
In addition to direct community engagement, the Mobile Unit Worker is responsible for the safe operation, daily setup, basic maintenance, and logistical coordination of the mobile medical unit. This includes ensuring the vehicle, equipment, and supplies are prepared to support safe, effective, and reliable service delivery. The role involves outreach, health promotion, community connections, harm reduction services (including drug checking where applicable), and coordination with interdisciplinary team members to support equitable access to care.
Accountability
The Mobile Unit Worker is a member of the Hamilton Urban Core interdisciplinary team and reports to the Senior Manager, People and Operations, with overall accountability to the Executive Director.
Key Responsibilities
Community-Based Outreach & Engagement:
Provide mobile, in-person outreach and supportive responses in community settings to increase access to care.
Build trusting, respectful relationships with individuals and communities through consistent, non-judgmental engagement.
Support system navigation and facilitate warm connections to appropriate services, including primary care, health promotion, mental health and addictions supports, housing, income supports, and emergency services when required.
Client-Centered & Harm Reduction Support:
Deliver services grounded in trauma-informed, recovery-oriented, harm reduction, and culturally safe practices.
Respect client autonomy, dignity, consent, and self-determination, including the right to accept or decline services where capacity allows.
Provide practical support, health education, harm reduction supplies, and referrals aligned with client-identified goals.
Recognize and respond to the impacts of systemic barriers, discrimination, and social determinants of health on client experiences.
Screening, Observation & Risk Awareness:
Conduct non-clinical screenings, observations, and basic assessments (e.g., safety, environmental, and social needs) in accordance with organizational policies and role scope.
Identify potential risks or escalating concerns and promptly consult with clinical team members or supervisors as required.
Support de-escalation and crisis stabilization using least-intrusive approaches and established protocols.
Collaborate with emergency services or crisis supports when risk thresholds are met, following program guidelines.
Mobile Unit Operations & Logistics:
Safely operate the mobile medical unit in accordance with transportation regulations, organizational policies, and safety standards.
Complete daily setup, takedown, cleaning, restocking, and basic equipment checks of the mobile unit.
Monitor supplies and report maintenance or equipment issues promptly to ensure continuity of service.
Support safe storage and handling of materials, including harm reduction supplies and confidential records.
Coordinate and support mobile drug checking services.
Team Collaboration & Documentation:
Work collaboratively with interdisciplinary team members to coordinate care and outreach activities.
Participate in team meetings, case discussions, and program planning as appropriate to the role.
Complete timely, accurate, and standardized documentation to support service delivery, reporting, and quality improvement.
Contribute to a respectful, inclusive, and psychologically safe team environment.
Equity, Access & Cultural Safety:
Deliver services that are inclusive, culturally responsive, and accessible to diverse populations, including Indigenous and Black populations, racialized communities, newcomers, individuals experiencing homelessness, and people with lived/living experience of mental health and substance use challenges.
Work within an anti-racism, anti-oppression, culturally sensitive, and language-appropriate framework.
Safety & Staff Well-Being:
Follow established safety protocols for mobile and community-based work, including situational awareness, communication procedures, and de-escalation techniques.
Participate in training related to personal safety, trauma-informed care, harm reduction, and critical incident response.
Practice self-awareness and seek supervision or support when working in emotionally demanding situations.
Other duties as may be assigned or delegated.
Qualifications
Required:
Minimum two (2) year Community College Diploma in a health, social services, or related field.
Minimum one (1) year of experience in a community-based, outreach, health, or social services role.
Valid Ontario G Driver’s License with a clean driving record.
Strong interpersonal, communication, and problem-solving skills, with the ability to engage respectfully with people from diverse backgrounds.
Ability to organize work, prioritize tasks, and work independently and collaboratively in a fast-paced, community-based environment.
Demonstrated ability to communicate in a culturally safe, inclusive, anti-oppressive, and trauma-informed manner.
Knowledge & Skills:
Knowledge of community-based outreach, health promotion, crisis response principles, harm reduction, and service navigation.
Working knowledge of Ontario social support systems (e.g., Ontario Works, ODSP) and local community resources.
Understanding of trauma-informed care, recovery-oriented practice, psychosocial rehabilitation, and harm reduction principles.
Familiarity with relevant legislation and privacy requirements (e.g., PHIPA, Health Care Consent Act) as applicable to the role.
Ability to remain calm, use sound judgment, and respond appropriately in high-pressure or unpredictable environments.
Strong teamwork skills and ability to contribute positively within an interdisciplinary setting.
Preferred / Assets:
Experience operating and maintaining large vehicles or mobile service units.
Experience working with underserved, marginalized, or equity-deserving populations.
Familiarity with mobile health, outreach, or community-based service models in Ontario.
