Full Time - 37.5 Hours/Week

Salary Range: $111,792.56 - $125,000

As an inner-city health Centre our client populations include individuals and families with chronic and complex health and social conditions, many of which are living in poverty.

The Nurse Practitioner will join our dynamic inter-professional team in providing holistic, comprehensive primary health care services to individuals, families, and communities in the inner city. A Registered Nurse, Extended Class, in good standing with the Ontario College of Nurses, will use advanced assessment, physical examination, therapeutic and diagnostic skills to treat, counsel or refer clients. Using a collaborative, shared care approach, the Nurse Practitioner will work with other team members in health promotion and disease prevention which may include individual, group, or community interventions.

The Black Health Nurse Practitioner functions as a member of the primary care team in the provision of primary health care services to residents of the community, with a strong emphasis on illness and injury prevention and health promotion. The Black Health Nurse Practitioner will work within the interdisciplinary team at the Centre to address individual and community health issues including providing holistic assessment of and care for patients and families with a focus on health promotion, disease prevention, health maintenance and rehabilitation

Key Responsibilities

  • Conduct comprehensive health assessments, provide diagnoses, develop and implement treatment plans, and deliver health education and counseling in accordance with the College of Nurses of Ontario standards for Nurse Practitioners (Extended Class).

  • Build trusting relationships and collaborate effectively with Black and racialized populations to identify and address community needs and barriers to access.

  • Collaborate with the interdisciplinary team and external service providers to coordinate and manage client care, including chronic disease monitoring, episodic care, follow-up, case management, and referrals.

  • Facilitate timely referrals to specialists, hospitals, and community resources as required.

  • Support the development and delivery of a primary care-based Sickle Cell Disease (SCD) clinic serving patients in the Hamilton region.

  • Work collaboratively with Ontario Health Teams (OHT), McMaster, and Hamilton Health Sciences (HHS) to deliver community-based care aligned with the Ontario Health SCD Quality Standards.

  • Integrate primary care with psycho-social supports and system navigation, using trauma-informed, anti-racist, and culturally safe approaches in partnership with community organizations.

  • Build and strengthen pathways between hospital and community to support holistic, client-centered care, with a focus on social determinants of health and existing SCD care structures.

  • Participate in shared care models with HHS's dedicated SCD Centre to ensure continuity of care—particularly following hospital discharge, during acute pain crises, and during the transition from youth to adult care.

  • Contribute to the development, delivery, and evaluation of health promotion, education, and counseling programs targeting individuals, families, and communities.

  • Ensure high-quality clinical care through accurate documentation, chart reviews, case conferences, case management, and timely responses to client inquiries.

  • Support the planning and evaluation of programs, workshops, and screenings by identifying and responding to the unique health needs of Black clients and other marginalized groups.

  • Engage in community outreach, including off-site and mobile care delivery, to improve access to primary care services within the Black community.

  • Provide supervision and mentorship to students, graduates, and learners in alignment with the Centre’s educational commitments.

  • Contribute to the development and improvement of clinical protocols and procedures, participate in committee work, and assist in maintaining clinical environments including monitoring controlled substances.

  • Maintain professional competence through ongoing education, including peer collaboration, literature review, and participation in relevant conferences or training.

  • Practice within a framework of anti-racism, anti-oppression, and cultural humility, ensuring services are culturally responsive and language appropriate.

  • Actively participate in interprofessional care delivery, reflecting the Centre’s core values and the Community Health Centre (CHC) Model of Care.

  • Provide backup clinical coverage for other nursing staff as needed.

  • Participate in interdisciplinary case conferences to ensure coordinated care.

  • Support Centre-wide initiatives, including client-focused events, community engagement, and organizational activities such as the Annual General Meeting.

Qualifications and Requirements

  • BScN with Nurse Practitioner certification; registered with the College of Nurses of Ontario in the Extended Class.

  • Broad-based clinical nursing experience in primary health care or community-based settings.

  • Strong clinical judgment, assessment, and problem-solving skills.

  • Excellent communication and interpersonal abilities, with a collaborative approach to care.

  • Demonstrated interest and experience in working within an interprofessional team model.

  • Experience providing care to underserved populations, including newcomers, isolated seniors, street-involved youth, individuals living in poverty, people experiencing homelessness, and those with mental health and/or addiction challenges.

  • Knowledge of the Community Health Centre model and its application in urban health settings.

  • In-depth understanding of the social determinants of health and the systemic barriers affecting African Canadian and Caribbean communities.

  • Demonstrated knowledge, experience, and cultural competence in working with individuals of African descent.

Schedule:

  • 8 hour shifts

  • Monday - Friday 9:00 am - 5:00 pm, 1 day per week 12:00 pm - 8:00 pm, occasional Saturday 9:00 am - 2:00 pm
