Inclusive, culturally relevant, people-centered care for Hamilton’s diverse community.

Hamilton Urban Core Community Health Centre provides accessible, high-quality care to support individuals and families across the city.

Our Mission: Strong Core, Healthier Lives.

Since 1995, we’ve served Hamilton’s urban core with compassionate, inclusive care that meets the needs of every resident.

Healthcare That Listens, Respects, and Responds.

We believe that everyone deserves care that respects their lived experience. At The Core, we provide primary health, mental health and addiction services, and community health promotion services that prioritize dignity, equity, and trust.

We know that barriers like racism, ableism, and economic hardship impact well-being — and that by focusing on those most affected, we help everyone thrive.
